The value of Nurse Practitioners and Physician Assistants to the Post-Acute and Long-Term Care community cannot be over stated. We believe that excellent care can be achieved when a physician and an NP or PA collaborate successfully. To that end we are looking for practitioners who love the Post-Acute and Long-Term Care environment and wish to be a part of a strong clinical team.
Collaboration and Support
We recognize the unique skills and experiences that nurse practitioners and physician assistants bring to SilverSage. We value close, ongoing, everyday inter-professional collaboration and interaction with colleagues. SilverSage practitioners appreciate being part of a team, and appreciate the opportunities to learn from and to teach others. We have a collaborative culture, and we foster collaboration by affording adequate time to spend face to face with colleagues every day. In addition, we promote collaboration through ongoing training and communication, along with regular conferences and face-to-face meetings with colleagues working in other facilities. We also offer one on one coaching and mentorship to all our employees.
EMR (Electronic Medical Records)
We know that the biggest drain on your time and energy is paper work. Although we cannot relieve you of this burden completely we are going to do everything we can to reduce the frustration. We chose a highly customizable and user friendly EMR with the goal of improving efficiency without added EMR stress. We have support staff ready to answer your questions and assist you with understanding and learning to use the EMR rapidly.
Billing & Coding
We provide billing and coding services and assistance and oversight of documentation. In addition, we provide a laptop for your clinical work and remote access to clinical records, as well as staff to support and help you along the way.
SilverSage recognizes the unique setting that is Post-Acute and Long-Term Care. We know that for you to give excellent care in this setting you must be comfortable in the environment and knowledgeable of the rules governing that environment. As a result, we place great emphasis on education not only to promote your comfort level in the environment, but to help you excel in it.

Safe Transitions of Care
Finally, your job is not only to collaborate with your Supervising Physician, and to be a partner with patients, families, and Long-Term Care facilities, it is also to be a bridge between hospitals and your long-term care facility. We know that as a vital part of a medical team, you have the knowledge and ability to improve safe transitions of care for everyone involved. This may include medical home visits in order to ensure a safe transition home and proper follow-up to ensure the best outcome for all involved. Bridging this gap is an invaluable service that you can provide.
